Registration for the 2018-2019 school year is now open on Final Forms. All parents of students in grades pre-K through 12th grade need to log into Final Forms to review, update, and sign all of the forms. For students in grades pre-K through 6th grade a student signature is not required. For students in grades 7 through 12, a student signature is required on all forms once a parent has signed all the forms. This requires a student in grades 7 through 12 to have an email address. Please use the school student email address (ex. 31jsmith@pettisvilleschools.org). The Final Forms login page can be accessed from the Pettisville School Website under the District Tab or the Quick Links.
For students in grades 7 through 12 planning on participating in a sport for this coming school year, a parent needs to complete this process as soon as possible by selecting the sport(s) in Final Forms, reviewing and updating forms, and signing all of the forms. Athletes will also need to have a completed athletic physical prior to the first day of practice for that sport. This is especially important for those participating this fall since practices for fall sports begin before school starts. The first possible date for fall sport practices is August 1. Once you have selected your sport(s), updated and signed all the forms, you will be able to print off the physical form (PPE Medical) to take to the physicians' office. If you have not already scheduled a physical appointment with your physician, please do so as soon as possible.
After your physical appointment, please make a copy of your completed physical for your records and return the completed physical to the school office. You may scan and email the completed physical to Brian Leppelmeier at bleppelmeier@pettisvilleschools.org as well.
For any questions please contact Brian Leppelmeier at 419-446-2705 or at bleppelmeier@pettisvilleschools.org
